Sponsor/Sponsorship Ad Form
Please fill out the following form to have your Sponsorship Ad created and displayed. Sponsorship Ads are paid for on an Annual basis and must be renewed during January of each year for the Sponsor/Sponsorship Ad to remain on the CSBA Website (Note: Please scroll down to the bottom of the form to see special instructions for Sponsor/Sponsorship Ad renewal).
PLEASE NOTE: As noted above, Sponsorship Ads are paid for on an Annual basis and must be renewed during January of each year for the Sponsorship Ad to remain on the CSBA Website. To renew a Sponsorship Ad, please fill out the form and indicate the following information: 1.) Under “Ad Info” >> “Which Ad did you purchase” please select your ad size to reaffirm your ad size 2.) Under “Ad Info” >> “Do you need an ad designed” select “No” and 3.) Under “Any other special requests or instructions” specify “No change to current ad”. Payment is made via PayPal (by choosing Renewal Ad size in the “Sponsor Options” selection box) or by sending a check to the CSBA Treasurer and following the instructions included above.
