Forms for keeping track of hives, queens, eggs, etc

Hi,
Does anyone know where I could find some forms for keeping track of our beekeeping hobby?  Things like when the queen was released, when the first eggs were hatched, when we first installed hives, swarms, etc?  I can make one, but would rather not reinvent the wheel, so to speak. Or at least I could get ideas of what has worked for others since husband and I are so incredibly new at bees.

You can go to HiveTracks.com, and they have an online hive inspection report that's good.  You can enter stuff with a smartphone/ipad right at the hive, or later.  Recommend you take a look anyway because it's other functions are really cool!  I started with the Dadant hive inspection form, it's really cheap for a pad of them, then I just made copies.  There are other good ideas, many on beekeeping blogs like http://www.littlehouseonthebighill.com/beekeeping/index.php?detail=129 which I've taken and customized for myself.  It gives you lots of reminders about things you should be looking for when you open up your hives, and being new at beekeeping myself, I needed all the reminders I could get.
